SSB PIQ Form Builder — Fill & Download Your Personal Information Questionnaire

Every SSB candidate fills a two-page Personal Information Questionnaire (PIQ) on Day 1 of reporting. It is the single source your Interviewing Officer (IO), the psychologist and the GTO all cross-verify against every answer you give across the five days. Use this free, browser-only builder to draft your PIQ in advance, catch inconsistencies early, and carry a clean printed copy.

Everything stays on your device. This tool does not send your data anywhere. It auto-saves your draft to your own browser's local storage so you can come back later, and never to any server. Use Clear all fields below to delete the saved copy.

How to make your PIQ work for you

  • Fill this online copy at least a week before your SSB and re-read it every day.
  • Every claim on the PIQ is a probe point for the IO — do not list a hobby or a POR you cannot defend for 5 minutes.
  • Cross-check names, dates and percentages against your mark sheets. A wrong year is a red flag.
  • Keep one line = one concrete achievement (year, role, outcome).
  • Print two clean copies on A4. Do not fold the printed sheet — carry it in an A4 envelope.
  • The SSB centre may hand you their official form on arrival — use this draft as your source when filling that form under time pressure.
